CDSS Alert Management Console

Aim — To work a live Clinical Decision Support System alert queue as the on-call clinical pharmacist — investigating each alert against the patient record, judging its true clinical severity, taking the correct action, and managing alert fatigue without dismissing the alerts that matter.

Your shift

You are the on-call clinical pharmacist covering the CDSS alert queue at CityCare Digital Hospital. Alerts arrive as prescribers write. Your job is not to clear the queue — it is to make sure the alerts that matter are acted on, and that the ones that do not are dismissed for a defensible reason.

  1. The severity the system assigns is not always the clinical severity. Some alerts are over-graded; at least one is under-graded and will harm a patient if you trust the badge.
  2. Nothing in the patient record is shown until you open it. Allergies, current medicines, labs and conditions each need a click — and what you open is recorded.
  3. Deciding an alert without opening the record raises your Alert Fatigue Index. Doing it repeatedly is how real pharmacists miss real harm.
  4. The Patient Safety Index falls only when a decision could actually harm the patient in front of you.
  5. The shift lasts 20 minutes. Alerts left unhandled when it ends are scored as dismissed without review.

At the end of the shift the console advances 24 hours and shows you what happened to every patient.
